We are able to offer and administer the Pfizer vaccine to anyone 12 years of age and up. The patient must have valid government-issued photo ID to receive a vaccination.
All minors must be registered and accompanied by their parent or legal guardian, and parent/guardian must have proof of legal charge and they must provide permission. Registration occurs onsite.
These vaccination sites are sponsored by the State of Florida and the Florida Department of Emergency Management. They are operated by Pulse Clinical Alliance and its employees. These sites are subject to change location and hours at any time. These sites are open to the public, weather-permitting.
